Self-pay

$75 per session paid at time of service.  I accept cash, checks, credit cards, debit cards, and health savings account cards.

Aetna

I am no longer accepting new Aetna patients.  But wait: what’s your deductible?  It’s huge, right?  Which means you’ll be paying for it anyway.

So come see me. I’ll give you a receipt, and you can send it to your insurance company to apply toward your deductible.  Or you can use your health care spending account.  The good news is that you don’t need a doctor’s referral to see me.

Blue Cross/Blue Shield

I am no longer accepting new Blue Cross/Blue Shield clients.  But wait: what’s your deductible?  It’s huge, right?  Which means you’ll be paying for it anyway.

(Besides which, BCBS bills through Premera and they now have a pre-authorization process, which means that whatever number of massages you think have, you will will be authorized for 6.  I guarantee.)

So come see me. I’ll give you a receipt, and you can send it to your insurance company to apply toward your deductible.  Or you can use your health care spending account.  The good news is that you don’t need a doctor’s referral to see me.

Cigna

I am no longer accepting new Cigna patients.  But wait: what’s your deductible?  It’s huge, right?  Which means you’ll be paying for it anyway.

 First Choice

I am accepting new First Choice patients.  You will need to bring with you a referral from your doctor (MD, DO, or ND), chiropractor (DC), physician’s assistant (PA) or nurse practitioner (ARNP).

Group Health/Kaiser

I am not in network with Group Health/Kaiser, but we can work together as an out-of-network provider.  You will still need a referral from your doctor (MD, DO, or ND), chiropractor (DC), physician’s assistant (PA) or nurse practitioner (ARNP).

 L & I: Washington State Labor & Industries Workman’s Compensation

I am registered as an L&I provider.

Bring a referral for massage written by your doctor (MD, DO, or ND), chiropractor (DC),  physician’s assistant (PA), or nurse practitioner (ARNP).

Also bring your case number and case manager’s name and phone number. You will not be required to pay at time of service.

Lifewise of Washington

I am no longer accepting new Lifewise clients.  But wait: what’s your deductible?  It’s huge, right?  Which means you’ll be paying for it anyway.

So come see me. I’ll give you a receipt, and you can send it to your insurance company to apply toward your deductible.  Or you can use your health care spending account.  The good news is that you don’t need a doctor’s referral to see me.

Medicare

Medicare does not cover massage.  Never has.

PIP claims (Personal Injury Protection, which is car insurance coverage for car accident injuries)

I am accepting new car accident clients.

Bring with you a referral for massage written by your doctor (MD, DO, or ND), chiropractor (DC),  physician’s assistant (PA), or nurse practitioner (ARNP).

Also bring a copy of your claim number, and information about the agent handling your case. You will not be required to pay for your treatment at the time of service.

Premera

I am no longer accepting new Premera clients.  But wait: what’s your deductible?  It’s huge, right?  Which means you’ll be paying for it anyway.

So come see me. I’ll give you a receipt, and you can send it to your insurance company to apply toward your deductible.  Or you can use your health care spending account.

Regence

I am not in network with Regence and cannot bill them.  I will give you a receipt to mail to Regence and get reimbursed if possible.  You will have to  pay at the time of service. But the good news is that you don’t need a referral to see me.

Uniform

I am not in network with Uniform and cannot bill them.  You will have to pay at the time of service.   I will give you a receipt to mail to Uniform and get reimbursed if possible.  You will have to  pay at the time of service. But the good news is that you don’t need a referral from a doctor to see me.

United

I am not in network with United and cannot bill them.   I will give you a receipt to mail to United and get reimbursed if possible.  You will have to  pay at the time of service.  But the good news is that you don’t need a referral from a doctor to see me.

Cancellation/No Show Policy

I reserve the right to charge a late fee for missed appointments.
