Self-pay

My fee is $75 per session paid at time of service.  I accept cash, checks, credit cards, debit cards, and health savings account cards.

First Choice

I am accepting new First Choice patients. You will need to bring with you a referral from your doctor (MD, DO, or ND), chiropractor (DC), physician’s assistant (PA) or nurse practitioner (ARNP).

PIP claims (Personal Injury Protection, which is car insurance coverage for car accident injuries)

I am accepting new car accident clients. Bring with you a referral for massage written by your doctor (MD, DO, or ND), chiropractor (DC),  physician’s assistant (PA), or nurse practitioner (ARNP). Also bring your claim number, and information about the agent handling your case. You will not be required to pay for your treatment at the time of service.

L & I: Washington State Labor & Industries Workman’s Compensation

I am registered as an L&I provider. Bring a referral for massage written by your doctor (MD, DO, or ND), chiropractor (DC),  physician’s assistant (PA), or nurse practitioner (ARNP). Also bring your case number and case manager’s name and phone number. You will not be required to pay at time of service.

Aetna, Blue Cross/Blue Shield, Cigna, Lifewise, Premera

I am in network with these but no longer accepting new patients. But wait: what’s your deductible? It’s huge, right? Which means you’ll be paying for it anyway.

So come see me. I’ll give you a receipt, and you can send it to your insurance company to apply toward your deductible or maybe be reimbursed.

Kaiser Permanente

I am not in network with Kaiser Permanente, but I can work as an out-of-network provider. You will still need a referral from your doctor (MD, DO, or ND), chiropractor (DC), physician’s assistant (PA) or nurse practitioner (ARNP).

Medicare

Medicare does not cover massage.  Never has.

Regence, Uniform, United

I am not in network with Regence, Uniform, United, or any other medical insurance plan that bills through Regence. I can give you a receipt to mail in and get reimbursed if possible. You will have to pay at the time of service, however.

Cancellation/No Show Policy

I reserve the right to charge a late fee for missed appointments.
